Dental insurance is a contract between the insurance company and the patient, not the dentist. Our dental office will complete and mail all dental insurance forms for you as a courtesy. We will do our best to help you receive the maximum benefit that your insurance plan allows. Your insurance company will reimburse you directly.
We make every effort to keep the cost of your dental care as reasonable as possible. To meet that goal, we collect for treatment at time of your visit. We are sensitive to the fact that different patients have different needs so we do provide various payment options. All financial discussions and arrangements are confidential.
We are participating members of several insurance companies, including, Ameritas, Principal, Physician’s Mutual, Cigna PPO, UHC PPO30, UHC Medicare Advantage, Envolve Wellcare, and Aflac. However, patients with these and all other insurance companies will be required to meet the deductible, co-payments and fees over the yearly maximum as well as non-covered services if applicable.
We will ask you to sign a financial responsibility statement reflecting acknowledgment and understanding of this policy at your initial office visit.
